Community Providers Inc. (CPI)
CVPH Medical Center is a not-for-profit, Article 28 institution governed by a voluntary Board of Directors comprised of 15 residents from the region. It is a subsidiary corporation of Community Providers, Inc. (CPI). CPI is a section 501(c)(3) organization and a 509(a)(1) holding company located in Plattsburgh, New York. CPI is a multi-corporate provider of health services. Along with CVPH Medical Center, other subsidiary corporations of CPI are as follows:

Elizabethtown Community Hospital
In 1993, CPI became the "sole member" of the Elizabethtown Community Hospital Corporation (ECH). ECH, established in 1926, is a not-for-profit, 25-bed hospital located approximately 40 miles south of Plattsburgh in Elizabethtown, New York. Historically, CVPH physicians have provided specialty medical care to ECH patients and patients in the ECH service area have been referred to CVPH for care.

Foundation of CVPH Medical Center
The Foundation of CVPH Medical Center, Inc., is a tax-exempt corporation established by CPI in 1989 to generate private charitable contributions to meet the general health care needs of the geographic region that CPI and the Medical Center serve.

Mediquest, Inc.
Mediquest Corporation is a for-profit corporation organized in 1985 for the purpose of developing other lines of business that are available to other entities as well as to the Medical Center. Mediquest is primarily a real estate holding company that owns several suites in adjacent medical office buildings and also is the parent company of EMT of CVPH, a freestanding, not-for-profit ambulance company.

Pine Harbour, Inc.
Pine Harbour, Inc., is a not-for-profit corporation founded in 2003 for the purpose of owning and operating a 66-bed assisted-living facility located at 15 New Hampshire Street in Plattsburgh, New York. CPI is an investor in Pine Harbour and has no operational responsibility for the facility.

Champlain Valley Health Network, Inc.
Champlain Valley Health Network, Inc., is a for-profit corporation created in 1996 to handle risk arrangements with health insurers. It currently provides physician-billing services and employs medical office personnel to support the practices of physicians employed by CVPH.
