Care After Discharge

It's been said that there is no place like home and certainly there are many places people would rather be than in a hospital. We recognize that and will work closely with you and your family to help you transition from CVPH to your home or another more appropriate setting as quickly, smoothly and safely as possible. One of our many caring and capable case coordinators will be glad to assist you. It is better for you to be out of the hospital and, during times of high demand, it is beneficial for our community that you are promptly discharged when hospital care is no longer necessary because other, much more ill people may be waiting for your hospital bed.
