CVPH Charity Care
What Is the Charity Care Program?
The Charity Care Program at CVPH is designed to help people who are unable to pay for services and do not have the means to do so. All patients receive medical care regardless of ability to pay. Persons must meet the eligibility requirements and all third party resources and non-hospital financial aid programs must be exhausted when applying for the program. This is not an insurance program, and is not meant to replace benefits that are, or could be, received from government-supported programs. Some of the government-supported programs are Medicaid, Child Health Plus, and Family Health Plus.
Who Is Eligible?
The Charity Care Program is designed to help people who otherwise have no means to pay. It allows patients to receive medical care regardless of ability to pay. Participants must meet the eligibility requirements and comply with the guidelines.
Who Is Not Eligible ?
Persons who exceed the income and other guidelines
Persons who owe less than $500
Persons who do not comply with Medicaid/third party guidelines
Persons who falsify their application or refuse to cooperate
What Is Not Covered?
The Charity Care Program does have limits to what it will cover:
Accounts under $500 (inpatient or outpatient)
Applications turned in late. You have up to 180 days after the service date to apply
Services billed from other physicians
Cosmetic surgery is not covered
Charges not incurred at CVPH
Prescriptions are not covered
How and When Do I Apply?
Application forms can be requested from the CVPH Business Office or picked up at the cashier's area at CVPH. Any person wishing to apply should contact the CVPH Business Office for an application or appointment. All applications must be sent to the CVPH Business Office. You can apply for the Charity Care Program any time prior to services being rendered or after services have been rendered. If services have been rendered, you only have 180 days to apply.

If you are unsure of how to obtain information of government supported Health Insurance, you can contact Upper Hudson Enrollment Services. Upper Hudson Enrollment Services provides assistance in applying for Medicaid, Child Health Plus, and Family Health Plus. Upper Hudson works with staff at CVPH to help community members' obtain/apply for government-supported health insurance.
