Social Service Case Management

The Resource Management Department at CVPH is comprised of a dedicated group of professionals who strive to make health care delivery as effective and seamless as possible for each individual admitted to the Medical Center. The nurses and social workers are thoroughly familiar with all the insurance programs and health provider programs offered in New York's North Country. They work closely with all members of the CVPH team, the patient and patient's family to help ensure the most appropriate care and resources are utilized for the patient's benefit.
