These steps apply whether you are being admitted to the hospital or coming for outpatient services at any of our locations.
Advance Registration is as easy as 1-2-3.
- Before you call be prepared with your
- Social Security Number
- Written Doctor's Order (We will need the test(s) \procedure being ordered and the diagnosis or reason for the testing)
- Health Insurance Card(s) or completed claim forms
- Call 562-CARE (2273) between the hours of 8 a.m. – 6 p.m., Monday through Friday to Advance Register. There is an automated system in place if all of our representatives are busy. You will be placed on hold and your call will be taken in the order it was received. Please remain on the line. You will be asked to verify all of your demographic information. The registrar that completes your Advance Registration will give you instructions of where and when you need to report for the services you will be receiving.
- On the date of your services, you will need to follow the instructions of where and when to report. Bring with you the following items:
- Written Doctor's order (we must have this in order to perform the testing needed)
- Health Insurance Cards, completed claim forms, authorization number or written referral from your insurance company if necessary. (Refer to your policy or call your insurance company.)
We will copy your insurance card(s), take your physician's order, and ask you to read and sign a release of information form.
