CVPH Health Plaza

The construction of a 19,600-square-foot primary care building at the CVPH Health Plaza is wrapping up.
On Friday, January 25th, we will be opening the newly constructed common corridor connecting CVPH Outpatient Services to Plattsburgh Family Health.
Patients will now be able to access the Diagnostic Center, Women’s Imaging, Blood Donor Center and Wound Care Center from both the north and south entrance of the new corridor. Temporary wayfinding signage will be in place to help identify CVPH services until the permanent signage is added this spring.
This is a big milestone in the 11-month construction project,. Thanks to everyone for your patience and understanding during this sometimes trying process as we work to improve access to services and primary care.
Hudson Headwaters will open Plattsburgh Family Health to patients in February 2019.
Parking at the Health Plaza
- For Renal Services: Use the North entrance on Degrandpre Way and park on the side of the building.
- For all other services: Use the South entrance on Plaza Boulevard to park. Enter the building through the Ambulatory Surgery Center. There will be wayfinding signs and volunteers to assist you. Wheelchairs will also be available. The entrance to Gastroenterology Associates will not change.

About the Project
Working together, Champlain Valley Physicians Hospital and Hudson Headwaters Health Network have established a Hudson Headwaters Federally Qualified Health Center (FQHC) in Plattsburgh to help increase access to primary care for North Country residents.
Using a $3.6 million matching grant from New York State's Capital Restructuring Financing Program (CRFP), CVPH has built a 19,600 square foot building adjacent to its Diagnostic Center located at the CVPH Health Plaza on Plaza Boulevard. Hudson Headwaters will operate in this space as Plattsburgh Family Health employing as many as seven providers and offering comprehensive high-quality primary care to patients.
