CVPH Health Plaza

The construction of a 19,600-square-foot primary care building at the CVPH Health Plaza is underway. The phased construction is planned to take twelve months, ending in March 2019.
Parking at the Health Plaza
- For Renal Services: Use the North entrance on Degrandpre Way and park on the side of the building.
- For all other services: Use the South entrance on Plaza Boulevard to park. Enter the building through the Ambulatory Surgery Center. There will be wayfinding signs and volunteers to assist you. Wheelchairs will also be available. The entrance to Gastroenterology Associates will not change.

About the Project
Working together, Champlain Valley Physicians Hospital and Hudson Headwaters Health Network will establish a Hudson Headwaters Federally Qualified Health Center (FQHC) in Plattsburgh to help increase access to primary care for North Country residents.
Using a $3.6 million matching grant from New York State's Capital Restructuring Financing Program (CRFP), CVPH will build a 19,600 square foot building adjacent to its Diagnostic Center located at the CVPH Health Plaza on Plaza Boulevard. Hudson Headwaters will operate in this space as Plattsburgh Family Health employing as many as seven providers and offering comprehensive high-quality primary care to patients.
