With your physician’s request, our therapists will evaluate you and recommend a treatment plan based on what you were doing before your illness and on your current function. The plan will be tailored to your particular situation and needs. A small gym located within the hospital may used in your treatment plan.
Certain medical problems also lead to difficulty swallowing, speaking or otherwise communicating. Our Speech and Language Pathologists work with patients on being able to safely swallow food and liquids; on understanding and expressing language; and on regaining cognitive abilities lost. They perform video fluoroscopic swallow studies with the assistance of radiology.
Our inpatient team works closely with the nursing and case management staff to prepare you for the next phase of your recovery. Whether it’s going home with some assistance, continued treatment at one of our outpatient settings or admission to a specialized facility, our therapists can help you get there.

Call Rehabilitation at (518) 562-4616 to schedule your appointment. When you call, one of our team members will be happy to make arrangements for your first visit. All we need is a prescription for rehab from your physician and your personal and insurance information.
For your first visit, please bring your insurance card, the referral from your physician, your co-pay, and any other pertinent medical information you might have. If you will be receiving therapy for your hip, knee or lower leg, it is recommended that you bring a pair of shorts to change into.
We ask that you arrive 15 minute early for your initial visit in order to complete some paperwork.
The Rehabilitation Services team plays an integral role in care of residents in the Skilled Nursing Facility and on the Sub-Acute Care Unit.
For patients in Sub-Acute Care, we tailor our treatment to what you will need to be able to do in order to be successful at home. We provide education for you and your family, so that you can continue to maintain your function at home.
For long-term patients, those who return a skilled nursing environment, we work to maintain the highest level of independence and quality of life while living in our facility.
Typically, the physical therapist will work with you on ambulation, leg strength, walking stairs, endurance, balance etc., while the occupational therapist will address your ability to do your self-care and other daily activities like dressing, bathing, toileting, folding laundry, kitchen activities etc. If you have problems with language or swallowing, the speech language pathologists will be included in the treatment plan.
Solo-Step is an overhead track and harness system designed to protect patients from falling during the rehabilitation process while giving them the confidence they need to break though their barriers in a safe environment.
Patients using the Solo-Step Fall Protection And Balance System during gait, balance, or strength training will be able to experience a level of independence they’ve never felt before.
