Footprints Forever

Commemorate the birth of a special baby by having his or her name, birth date and footprint engraved on a Corian® tile and displayed on a unique wall at the Alice T. Miner Center for Women & Children.  Parents, grandparents, godparents or other may wish to celebrate the special arrival and will receive a second Corian® tile as a keepsake.

Proceeds from this program will support important patient and staff education such as childbirth education classes; lactation education and support; as well as continuing education for our compassionate nursing staff.

A gift of $200 will purchase two Corian® tiles displaying the newborn's full name, date of birth and original footprint.  One is placed on a special wall in the Center for Women & Children and the other is mailed to the recipient of your choice.  Please complete this form and return to The Foundation of CVPH.  Please make checks payable to The Foundation of CVPH or complete the credit card form below.
